Chapters Health PACE, a complete healthcare solution that provides a continuum of care to participants at home and at the proposed 11,330 square-foot PACE Center, located at 5102 W. Linebaugh Avenue in Hillsborough County. Chapters Health has been authorized to become a provider of PACE (Program of All-Inclusive Care for the Elderly) services in Hillsborough, Polk, Highlands and Hardee counties. Chapters Health is completing the application process to serve the residents of Hillsborough County and anticipates enrolling participants in September 2011. Chapters Health is planning to expand PACE services to Polk County in the summer of 2012.
