This unique model of care helps people live safely at home with assistance from a team of compassionate healthcare experts. LIFE offers a total solution for older adults who experience health problems that limit their daily activities and for their family caregivers. By providing complete medical, health, and social services in a centralized location, as well as in participants’ homes, LIFE enables older adults to live independently. For the convenience of our participants, transportation is included. With the LIFE program, St. Mary Medical Center demonstrates its commitment to improving the quality of life for everyone in our community, including elderly Bucks County residents who need assistance to remain independent in their own homes.
