About Independence Health & Therapy
Our ADULT DAY HEALTH program is for individuals with a variety of diagnoses and ages. Socialization and therapeutic activities are provided for individuals who require assistance or that may otherwise be isolated. Our MEMORY CARE program maintains and improves cognitive functioning in a socialized setting. This program prevents premature placement in long-term residential care. Monday-Friday, 9am-4pm Door-to-Door Transportation Registered Nurses & Certified Nursing Assistants On-site Care Coordination & Medication Monitoring Respite for Primary Caregivers Caregiver Coaching We Accept: Medicaid Medicare Private Pay Commercial Insurance Uninsured
