Our Adult Day Health Center offers an array of supportive services designed to prevent or postpone the appropriate institutionalization of frail, homebound elderly or functionally limited adults Webster, Massachusetts. The adult day industry is comprised of two models of care, the social model and the medical model. The social model of care, are community-based group programs designed to meet the needs of functionally and/or cognitively impaired adults through an individual. Accord Adult Day Center, Webster, Massachusetts is certified to provide Adult Day Health (ADH) Services by the Commonwealth of Massachusetts, Department of Health and Human Services, Executive Office Of Elder Affairs and the State of Connecticut, Department of Social Services.alized plan of care. These structured, comprehensive programs provide a variety of social and other related support services in a protective setting during any part of a day, usually between four and nine hours per day. The medical model incorporates all the features of the social model but adds a wide range of health related services on site.
