Immanuel Pathways is an approved PACE (Program of All-inclusive Care for the Elderly) organization. We offer a complete health plan for eligible individuals providing a more personal and comprehensive way of getting health care. We want to help you continue living in your own home and community for as long as possible. Our comprehensive team of health care professionals helps coordinate your health, your connection with the community and the life you’ve built in your home. This program provides community-based care and services to people assessed to need nursing facility level of care.
