For 40 years, Sitrin's Adult Day Health Care program has been a popular spot for adults in the Mohawk Valley. Our innovative program offers the best of both worlds, providing assistance to those who need it but who do not require skilled nursing care. A great way to spend the day. Located within the Sitrin Health Care Center on Tilden Avenue in New Hartford, Adult Day Health Care provides flexible, individualized services during the day, making it possible for individuals to continue living at home while receiving the personal care, nutrition, and therapies they need at Sitrin. Just as importantly, the program gives older adults an opportunity to make friends, take part in activities, and enjoy life in a safe and supportive environment – one that brings a greater sense of well-being to participants… and their family members as well. Our Adult Day Health Care program offers a comprehensive array of services that enable adults to live as full a life as possible. Its programs are designed to meet the medical and social needs of adults. Medical Model Program - Along with having a Licensed Nurse on staff, the program is available seven days a week and includes meals and snacks, personal care assistance, medication administration, physician prescribed treatments, and therapy services, as well as recretional programs and community outings. This model is specifically designed for individuals who may have physical or cognitive conditions which require supervision and monitoring. Social Model Program - The social program, which is available on weekdays, includes meals and snacks, basic first aid, personal care assistance, medication assistance and supervision, along with recreational programs and community outings. This model provides individuals with an opportunity for social interaction with peers and care assistance while maintaining independence in the community.
