THE EAST HAMPTON ADULT DAY CARE MODEL IS A SOCIAL MODEL ADULT DAY CARE PROGRAM and provides services for Town Residents age 60 and older, who are frail, impaired, physically or cognitively, and capable of benefiting from socialization. The purpose our programs is to enable residents to remain at home in the community and offer respite for the caregiver. Our programs provide supervision and monitoring, nutrition, personal care, and socialization
