About St. Luke’s Home Adult Day Health Care Program
The mission of the Adult Day Health Care Program is to provide those who are entrusted to our care with a supportive, nurturing and family-centered environment that promotes wellness, independence and self-esteem, and that recognizes and celebrates the value of the individual. Our Adult Day Health Care Program is a medical model program, operated under the supervision of a registered nurse. Our program provides comprehensive healthcare with therapeutic social, educational and recreational activities. Our program enhances the lives of the frail elderly, the chronically ill and disabled adults while maintaining their residence in the community. At the same time, we can provide a period of respite for family caregivers.
