Home Avenue Adult Day Health Care is a day program providing a variety of health, therapeutic, and social services designed to serve the special needs of frail and elderly persons as well as adults who are disabled. Our therapeutic environment is designed to promote socialization and to maximize each individual’s potential for independence and self-esteem. Home Avenue ADHC also assist Family Members and Caregivers receive respite and prevent burnout.
