Grace Adult Health Care Center (ADHC) is an all-inclusive day clinic in Santa Clara, CA that provides the elderly population and other qualified persons with full range of health care, rehabilitative, social, psychological, related support services, balanced meals and door-to-door transportation. Grace ADHC is fully licensed by the California Department of Public Health and certified by California Department of Aging. Grace ADHC is an approved Community Based Adult Services (CBAS) provider. Participants spend quality time with people of similar age and background and participate in therapeutic recreational and social activities that encourage healthy, independent lifestyle – all under one roof and supervision of skilled and caring professionals. By attending Grace ADHC program a person could maintain or improve their health and physical abilities and avoid emergency rooms, frequent hospitalization, and/or premature institutionalization in a nursing home or other higher level of care. Our services are COMPLETELY COVERED for Qualified Medi-Cal Participants.
