Siouxland PACE, operated by UnityPoint Health - St. Luke's, is a Program of All-Inclusive Care for the Elderly. We help you live as independently as possible in your home and within the community through a range of services. The only program of its kind in this area, Siouxland PACE provides added peace of mind for our participants and their families. The program provides a unique opportunity for you to work with a team of PACE professionals committed to your health and well-being. As part of UnityPoint Health - St. Luke's, Siouxland PACE has access to a variety of your health care needs change. From home health to rehabilitation, our PACE team can coordinate a full-range of resources for you. The PACE center is an unique feature to the program and key to the success of the program. This center separates PACE from an ACO or MCO.
