About Elder Service Plan of CHA
The Elder Service Plan of CHA was established in 1995 to provide a new option for older adults. Over the past 20 years, we have provided support for thousands of Massachusetts adults and their families, and we would be proud to provide our services to you. We are part of the national PACE Program, Program of All-inclusive Care for the Elderly. These programs offer coordinated, interdisciplinary care for people 55+ who have medical needs but want to continue to live in the community for as long as possible.
