They are community-based programs that conduct medical monitoring under the supervision of a physician. A registered nurse monitors blood pressure, blood sugar, medications and other health conditions. We provide door-to-door transportation in modern, wheelchair-accessible vehicles. This allows those who need physical assistance or who are wheelchair-bound to leave their homes and participate in social and therapeutic activities. Dental, podiatry and optometry consultations are also available. Primary care continues to be provided by the registrantsâ€™ community physician.
